Grooming Consent Form

Please carefully read and accurately complete our grooming consent form.

If fleas are found on your pet, they will be treated with Capstar to kill all fleas immediately. A fee will be added to your bill based on the weight of your pet (up to $12 for pets over 25 pounds).

Please be aware that an additional fee will be required for pets who are matted ($35.00 per 15 minutes of specific de-matting time).

Sedation

A physical exam is performed on all pets that require sedation for a groom. If your pet has been examined by our veterinarians in the last year, the exam fee will be waived. If your pet has NOT been examined in the past year, then the cost for an exam will be $80.00. The sedation cost for a dog or cat is $125.00.
